Welcome! I understand that beginning supervised visitation can feel overwhelming. That's why I've designed a clear, step-by-step process to guide you. To begin, please complete the following:
Complete the Online Interview Questionnaire: Please complete the online questionnaire form. This helps me gather important information about your family’s needs and helps me to ensure a safe and successful visitation experience.
Review the Following Documents:
Supervised Visitation Guidelines: This document outlines the rules and expectations for supervised visits.
"Helping Children Prepare for a Visit": This document provides guidance for the custodial parent to help children prepare for visits.
"Tips for Having a Successful Visit": Advice for both parents on how to make the most of their visit.
The Concern Form: To be used to document any grievances or complaints about the visitation services.
Before our interview:
Photo Identification: Please email me a picture of your driver's license or photo ID, and your license plate.
Copies of Court Orders: Please email a copy of your most recent custody and protective orders.
Documents to Print, Sign, and submit or bring In-Person:
Service Contract: This document outlines the terms and conditions of my services.
Neutrality Agreement: This agreement states that I will remain neutral during the supervised visitation process.
Supervised Visitation Agreement: This agreement confirms your understanding of the terms and conditions of the visits and the provided guidelines.
What to Expect Next:
1. Initial Intake: Once your documentation is submitted, I will reach out to schedule an intake interview, either via phone or in-person, to discuss any specifics pertaining to your case.
2. Child Interview and Orientation (if applicable): If your child is old enough, they will also have their own orientation process before the first visit. I will schedule this separately with you and your child.
3. Before Visitation. Please complete the required paperwork and contracts and submit a copy via email, or bring these to our initial visitation. Payment is due on arrival from the paying parent.
